The Passport Foundation was created in 2017 to assist under-served and low-income communities, throughout Kentucky to help people of all ages live healthy lives.Â We do this by securing and providing funding for various initiatives and meaningful partnerships with service providers, corporations, community health organizations, other foundations, and educational institutions.
